Membership Application Request

Before completing application, please note: 

1.  Subsidiaries and divisions of applying companies must retain a separate membership.

2.  GMHA benefits and services will be applicable only to the company name indicated.

3.  All memberships will be renewed on an annual basis and will be renewed the month following the anniversary month.

4.  All membership applications will be processed in accordance with the Membership Ethics Review Committee (MERC) policies and procedures.

Supplier & Professional Service Companies:

  • Briefly describe type of business (type of services offered) and product (specify products, manufacturer and/or distributor)
  • Percentage of applying company's current business in the manufactured housing industry
  • Briefly state what benefit(s) membership in GMHA will provide your company
